By the time I finished filling out the survey written by a teenager, I decided to write one for folks who vaguely remember being a teenager.

In the Last Year:

1. Make a major career move? For the better?
2. Buy or refinance your house?
3. Spend more money on prescription drugs or recreational drugs?
4. Had surgery? Major, Minor, Elective, Cosmetic?
5. Bought or leased a car? Two? Three?
6. Used frequent flyer miles to travel for pleasure?
7. Used your frequent flyer miles to let someone else fly for pleasure?
8. Retire? Return to work?
9. Start a business?
10. Divorce or end a long-term (greater than 10 years) relationship? (Whose wanted out?)
11. Consider ending a long-term relationship, but worked it out?
12. Celebrated a relationship anniversary of greater than 10 years?
13. Began or finished a mid-life crisis?
14. Create a will, an advance health directive, prepay or plan for your funeral or other death related preparations?
15. Help someone else make plans regarding the end of their life?

6. How many cars, trucks and motorcycles have you owned?
7. How many other vehicles (boats, jet skis, airplanes, etc.) have you owned? What vehicles.
8. How conscientious are you about matters of health? (smoking, diet, exercise, etc.)
9. How important are the matters of health from number 8 to you?
10. In what ways do you think you have declined in the last 10 years? And in what ways do you think you have improved in the last 10 years?
